Cost-plus rules

Some deals pay a markup over ad spend instead of a per-lead payout (e.g. spend × 1.3). Write those rules here. A rule matches an ad-spend row when all its conditions are true; if two rules match, the lower prioritynumber wins. To change a payout over time, don't edit a live rule — start a new periodso last month's numbers never change.
